I agree to accept the equipment listed on this form as is and accept full responsibility for the care of this equipment and agree to pay this ski shop for any loss or damage resulting from the use of this equipment, other than that caused by reasonable wear.
I understand and acknowledge that the sports of skiing and snowboarding and the use of ski/snowboard equipment involves an above normal risk of injury and I am voluntarily participating in these activities with knowledge and understanding of the dangers involved.
I understand and agree that I am responsible for my own safety and accept the dangers that are inherent in skiing and snowboarding including injuries which can result from variations in terrain; surface or subsurface snow or ice conditions; bare spots; rocks; trees and other forms of natural growth or debris; collisions with ski lift towers and their components, with other skiers or snowboarders, or with properly marked or plainly visible snow- making or snow - grooming equipment.
I understand the Alpine ski equipment forms a part of or all of a ski-boot-binding system which will not release or retain at all times or under all circumstances where release or retention may prevent injury to any part of the skier's body, nor is it possible to predict every situation in which it will release or retain and it is, therefore, no guarantee of safety to any portion of my body. If snowboard or Nordic equipment is being furnished, I understand that these systems are not designed nor intended to release in the event of a fall or other type of accident, that it functions differently from typical Alpine ski equipment and that its use does not protect against nor reduce the risk of any injuries to any part of the user's body.
I also understand that the risks which skiing and snowboarding present to me and to others may be reduced by taking lessons, by following "Your Responsibility Code," which is posted at most ski areas, and by using common sense. I further understand that a leash or other runaway prevention system should be used with this equipment at all times, including while riding lifts, in order to reduce the risk of injury to others.
Recognizing all of the above and in consideration for my use of the equipment listed on this form and my use of the ski area. I agree for myself, my heirs, my personal representatives, and assigns that I assume all risks of accident and/or damages and/or injury to my person or property, including, but not limited to, any injuries arising out of my use of the ski or snowboard equipment or facilities, or ski lifts. I hereby release Indianhead Mountain Resort Inc., its employees, and agents from all claims for personal injury or damages sustained while on the premises of Indianhead Mountain Resort Inc., except if resulting from an intentionally caused injury, gross negligence, or willful and wanton misconduct on the part of the persons and entities released. I further agree to indemnify and hold harmless the persons and entities released from all claims which may be brought by others arising out of any personal injury of damage I sustain while using the equipment of facilities described in the agreement.

I ALSO DECLARE AND REPRESENT THAT I AM 18 YEARS OF AGE OR OLDER, OR THAT I AM LEGAL GUARDIAN OF THE PERSON RENTING AND USING THE EQUIPMENT LISTED ABOVE.
